An etiquette question: would you really include both "sir" and "dr" when addressing someone?
Because it's just "Sir Anthony Hopkins", not "Sir Mister Anthony Hopkins". Maybe it would just be Sir Nytik.
Perhaps it depends upon the context. In the office/hospital/wherever the Doctor works, he should be referred to with the title of "Dr."
Outside the profession, however, "Sir" is acceptable.
