My personal best is 99 playing the Sicilian Scheveningan. But they blundered their queen into a pin (which can happen in this variation if you aren’t familiar with it) ... and that made it easier to play accurate moves in the rest of the game.  https://www.chess.com/analysis/game/pgn/4WCL6ZnwHx

Yes 100% is possible, but the most I have got past book moves was 99.5%. Obviously others have got more than that though.

Yes but why on move 5? I am 1331 so maybe that happens more at the higher level but it just seems odd

It depends. I too had a game drawn under 5 moves . It was in a tournament and I was too tired . And perhaps my opponent too wasn't interested either to play . So we agreed for the half point .

Sometimes when you are in a tournament , and there's a situation where both you and your opponent might want to hinder the chances of a third person progressing further , you may want to agree to a draw
