What is the correct procedure to end a game that involves perpetual check? 

Should play continue until one of the opponents times out? 

Who should declare a draw?  If a player declares a draw, is the opponent obligated to draw?

Repeat the position 3 times and you can claim a draw, they will have no choice. Over the board I think you have to call the tournament director before your move which will repeat the position for a third time. Here you just hit the claim draw button (online games) or the draw button in live games and it will be automatic. Alternatively, just announce the perpetual and offer a draw after your move.

Suppose my opponent does not accept a draw with perpetual check?  Some games involve different moves that don't repeat three times in succession.

They don't have to be in succession, the position just has to be repeated 3 times in a game, check my last online game.
