Yes there is a CM but few use or recognize it. There also are various Class players from A-J in the USA. I'm not sure if FIDE rates players below the CM level.

Because the girl ones are easier to get, but girls get men title as well as they get stronger (WGM is easier than IM, so they get WGM first and then as they get stronger IM, then GM etc).
