Where did I go wrong in this game?
I think you pretty much nailed your problems in the game. His two center pawns gave him control of the center. Your fianchetto seems to be a waste of a move and goes against the purpose of your opening. Also, the opening is usually the exchange of your center pawns for one of the opponent's center pawn. It aims for quicker development in exchange of a pawn.

My aim was to keep the two bishops but white's knights got very good support points at f5 and e5. Also, he had two center pawns to my zero. I was counting on my c5 pawn to provide some support in the center. Maybe fianchettoing my queen's bishop wasn't a great idea. What should have been my basic plan from the opening. I face this opening quite often and struggle to fight against it.
