Why are "miss" and "inaccuracy" used interchangeably by the game review?

I've noticed this when reviewing games. A move will get a ?!, and the coach will describe it in a way that clearly states it was a miss. I've even seen mistakes (?) described in ways clearly implying that they were misses.

It doesn't really matter a whole lot, but it confuses me. Why have a "miss" category if most "miss" moves aren't even counted as such?
