You should be able to use the Support option in the menu to request an appeal. That said, most appeals are not granted.

I am writing to appeal the blocking of my account Elisey_Ananiev. I understand that my account has been blocked due to suspected Fair Play Policy violations.
I want you to know that I have been playing fair. My recent sharp rise in rating is due to intensive live chess work: I've been practicing with a coach, participating in real tournaments (including the Ukrainian championships), which has allowed me to improve my game significantly. Changing my opening repertoire (switching to e4 and the Italian game, and gambits) was part of this work. I worked on understanding the positions, not just memorizing moves.
I did not use any programs, engines, or help from others. I ask that you recheck my account given these circumstances. I value my honest name very much and would like you to review my situation.
