I'm IM Lars Ootes. I need title on chess.com but I don't know how to apply for it.

Please inform me what is the procedure to procure the title as preceding the name of the titled players? 

E.g GM Anish Giri.

I too would like to have  IM Lars Ootes in front of my name. Please explain what documents do I need to verify my identification and procure this?
