Chess etiquette question

Is there some etiquette out there for taking a draw when the opponent slips up on the placement of a piece?

If a piece slips for me, then depending on the situation I will either keep playing or resign.

This situation came up yesterday when an opponent moved their queen to a wrong square and I  captured it.

The opponent quickly and repeatedly offered a draw which I declined, three times in a row. 

They then made an off handed remark before resigning.  

Was I supposed to take the draw offer?

It sucks to suck. I never offer draws unless I think the 60 move rule is in effect.
