Forgetfulness in chess

Yesterday Radjabov forgot his move order and got beat by Ivanchuk. Have you ever forgotten a move order? Please tell us your mistakes!

Last summer I played a chess game in Northumbria. Had the black pieces, and decided to play the Caro-Kann for the first time in my life, got to the second move, could not remember a thing from my preparation, and hence moved my king's knight to f6. At first my opponent thought I had created a novelty- in reality I ended up playing a really bad Alekhine's defence and losing......hilarious.

Yes. I write my moves on a piece of paper. It is entirely possible for me to misplace a move.

I'd imagine it'd be pretty hard to forget the only possible point of 1...c6...to prepare ...d7-d5!
