1. Rc8+ Rxc8  2. Qxc8+ Rf8  3. Qe6+ Kh8  4. Qf7!

This was my most recent game and I did not see 4. Qf7.  Again, I was running low on time and I saw the check on e6 but not the follow-up.

My respect for the elite players just keeps rising.
