You are now in the homescreen!

By the way I have 3 Chess.com accounts!

Go where to left of the page, a site located called go to Chess today found at Community with a Green Chess pawn, Once you found it click on it but if you can find it click community and scroll down until you located it! (Under content) Scroll down when you are on the page look to the right whiling scrolling down You will see something label survey

  Choose an option and your awards should be having the Achievement Survey Says

Note: if it doesn't unlock Report Chess.com About it and they will help you get your achievement!
