For basic information about medals, see https://support.chess.com/article/696-what-are-trophies.

Different tournaments award different medal styles. I have found five styles of ready-made medals and the types of tournaments that award them:

Jagged Edge with Number
Live arena tournaments directed by a member in a club or directed by chess.com.
Smooth Edge with Number
Daily or live Swiss tournaments directed by a member in a club.
Smooth Edge with Pawn
Live Swiss tournaments directed by chess.com.
Smooth Edge with Rook and Pawn
Daily tournaments directed by a member in a club or directed by chess.com.
Smooth Edge with 960
Daily 960 tournaments only. (Live 960 tournaments award jagged edge medals with numbers.)
Custom
When creating a tournament, directors can accept the ready-made (default) medals or add their own images for the top three placements in a tournament. Examples are shown here.

If a number appears in the lower right of the medal, the recipient has earned that medal more than once. It indicates the number of times that medal has been awarded to the recipient. Custom images are also included in the applicable stack and total number of the default medal had the tournament director used that default medal instead of the custom image.
