WHY..WHY. ??? Often a lost is traced back to a simple issue..overlooking the obvious. AN exchange not made, or maybe you had an opportunity to force a material win; how about winning a piece outright but it's not done. Memory when pulled by many decisions will blank out( we all done that!)and the simple rather than the complicated results.Tension and stress compounds the problem. How do we overcome this daily chess problem? PENCIL and PAPER. Write down each move w/ your idea's and refer to it during the game and maybe reflect back by playing the game over a few times. Having it on paper will help your game.Very much like those reminders w/ see each time we hit the ice-box. Write it down.....Write it down!!
