5.Nc3 is the only move to keep the gambit moving.
5.d4 c5 6.dxc5 d5 7.e5 Ng4! is very troublesome for White.
5.Nc3 Be6, or 5...Bg4 6.f3 Be6 followed by ...Kg8 and eventually ...Bf7 is the computer's preferred way to meet 5.Nc3, which means that almost certainly it is not best!

Maybe but the final position on my 1st diagram seems to be quite pleasant for black (except for the rook blocked on h8).
Cochrane gambit: 1. e4 e5 2. Nf3 Nf6 3. Nxe5 d6 4. Nxf7 Kxf7.

5. Nc3 (-0.22) Bg4 6. f3 Be6 7. d4 Kg8 8. Be3 c5 9. dxc5 Nc6 10. cxd6 Bxd6 (-0.47)
It's quite complex. Black preferes to give 1 additional pawn to White for the sake of quick development.
2nd test line with 5...c5:
6. Bc4+ (-0.04) d5 7. Nxd5 Be6 8. Ne3 Bxc4 9. Nxc4 Nc6 10. d3 b5 (+0.25)
It's not so good for black.
