Think of it like stage directions for an actor:

To {emphasis on 'be'} be or {pause here, then lower your voice} not to be; {shout} that {back to normal tone} is the question.

A precursor to CSS was Rich Text Format - which was a similar mixture of instructions and content. e.g.

The quick brown FOX jumps over a lazy dog.

would be stored as something like ...

{Font Name=Times New Roman}{font size=12pt}The{Font Name=Arial}quick {colour=dark red}brown {colour=black}{case=capital}fox {case=lower}{effect=superscript}jumps {effect=normal}over a {effect=underline}lazy {effect=no underline}{effect=bold}dog{effect=bold off}.

(If you actually type this into MS Word and save it as RTF, you'll notice that it's a lot more complicated in practice!)
