I send requests to people I know in real life and accept the same. Beyond that, I send few and accept fewer. After consistent, interesting and respectful conversation in the forums, I may send or accept a request with someone whom I don't know IRL.
If a request pops up when I want to use the iOS app, I will refuse almost without looking at who it is from. Other requests from strangers may sit in my inbox for a few days or weeks before I take action.
