Finding the form to get a FIDE ID

Hi, I wanted to start playing in FIDE tournaments, however, I need a Fide ID, It is suposed to be on the US chess federation website but i cannot find it... can some give me the link/ tell me how to get it?

Should be included as part of USCF membership but you shouldn't have one until you play in FIDE rated section, this is how we do it Canada would be the same for US I believe 

for your rating it will be:

180 + 0.94 × FIDE if FIDE ≤ 2000
20 + 1.02 × FIDE if FIDE > 2000

Unless you're going to be travelling to another country to play a FIDE tournament, you don't need the ID. The first time you play in a US event that is FIDE rated, you'll automatically get the ID if you score in the event.
