You need to play FIDE rated over-the-board tournaments.

CM = Your FIDE rating is 2200+

FM = Your FIDE rating is 2300+

IM = Your FIDE rating is 2400+

GM = Your FIDE rating is 2500+

These are FIDE titles. That means that they are awarded by FIDE (the world chess federation). As Deranged has already mentioned, you need to play FIDE rated games (with slow time controls, not blitz/rapid) over the board and aquire a mininum rating. This and the additional criteria are defined in the FIDE handbook (see https://www.fide.com/fide/handbook.html?id=10&view=category for the details). For IM and GM, you usually have to collect 3 norms at tournaments with titled and international participants. For FM, you only need to boost your rating to 2300, and for CM, it's only 2200. When I started playing international tournaments, there was no CM title, because there was no FIDE rating below 2200. When you didn't deliver a 2200+ performance, you didn't even aquire a rating. Now it's quite easy to get a rating, but it requires a lot of work to get your rating above 2200. Beware that your opponents are also working on their skills, and they are utilising databases and books to prepare for each game.
