If your account was closed by yourself and you wish to reopen it, you may contact support to explain your case to them and they'll take care of it. You will get back a, b, c, d, d and f most likely.
If the account was closed for abuse / cheating, you should also appeal but I'm not sure if chess.com will agree to reopen the account. It'll depend on what extent you abused / cheated to. You will also most likely get back a, b, c, d, d and f.
If in any case you make a new account, your elo will be back to 400, 800, 1200, 1600, or 2000 depending on which option you choose, but forums, clubs, lessons & memberships will have disspeared.

d. Lessons you have learnt
