My current best method is to use my (Android) phone with the "Chessvision" app. It's free (probably because it's an low-low-key ad for their web site that processes entire books in PDF) .
Only tedious part is sending it to the PC -- not serious, and my method is "Share" to "Discord" on a #channel I use specifically for these -- then copy on the PC Discord app easily.
Hi, Chessvision.ai creator here - glad you like the app. For sending diagrams to your PC there is another method you can try. In the app, there is this Diagram Library that you can access from the Menu in the top-right corner. It contains all the diagrams you scanned and marked with the Star (there is a Star button just above the Analysis Board and clicking it adds the diagram to the Library). Then, on PC, open https://my.chessvision.ai in your browser, log in with the same account you use for the mobile app, and go to the Diagram Library tab which is synced to your Library in the mobile app - you can easily export/copy diagrams you want from there. Pkacprzak: Chessvision ai is a fantastic set of applications. I use the version adapted to Firefox, and the integration of your FEN conversion program and Stockfish. I normally have the chess engine turned off, while I mentally try to solve a puzzle. Please continue to improve your suite of apps, especially for ebooks, where the wealth of chess knowledge is waiting for any student of the game.
There are a number of mobile apps that do the job good enough for most puposes. Chessify and Chesssvision.ai are two such apps that perform well (I see that chessvision.ai is mentioned already here).
If you need such a tool in Mac or Windows, try ChessConverter.

i just wanna say it's so crazy how this thread went from people in 2013 saying "this would be really cool but probably not possible" to you saying in 2022 "hey guys, i made an app that does this!"

Go for it. Chess is played by millions of people, all over the globe. Tens of thousands of webpages exist that are chess related, or have chess diagrams. There will always be an interest for something like this.
