Regarding the 3 fold repetition rule:
1. Does the position need to come up for the 3rd time in a row or simply in the game? For example at move 14, 33 and 42?
2. Do the same pieces of a certain type need to be in the same place? For example, does the same rook need to be in the same place, or simply A rook. In other words, does the the rule apply if for example there are rooks and knights positioned on the same squares, but the actual pieces have switched?

1. No, the positions do not have to be consecutive.
2. The pieces all have to be on the same squares but they do not have to be the pieces that started on those squares. However, if castling was possible in one of the postions, then that would not be a repitition for a position where it was not possible, even with all pieces on the same squares.
