Your notes are quite accurate I feel. A shame you lost because you put yourself in a winning position. Too passive in certain positions, a bit cautious. And of course you should have taken care of that passed b pawn which promoted. Pawns close to promotion are exceptionally dangerous and there are often tricks and tactics around them. Try not to allow passed pawns, and if you do, try to restrict them from advancing as early as possible. Work on your tactics - as you say in the notes there were several 2-3 move sequences which you didn't go for which would have been good for you 👍

I think you're spending too much time evaluating your own games (which has value) when you could invest this time in to learning the foundations of chess strategy. I can't be sure because I don't know what studying you do, but I think you'd find saint louis chess club, kestony, or any of the other options for free youtube chess lessons very rewarding since you're already investing effort in to improving.
However, that being said, you clearly want to improve and are putting work in to that effect so you can also just keep doing what you're doing and that will be rewarding too. I personally use two chess websites, this one and lichess, I play casually here as it is less moderated (cheaters) and only play games I take very seriously on lichess. This lets me have fun playing in one spot where I don't crash my rating playing drunk or tired, and have a good point of reference for my maximum potential in ratings on the other. I use the same name on either website you can see the rating difference is like.. 700 points. Makes you wonder who's system is so flawed

- I can’t take credit for annotating the game I asked for some help. It’s been about 3 weeks of playing with already knowing how the pieces move and knowing to go after the center. I’ll be working on my foundation and tactics because when the game gets passed a few moves I try to control the center and hope for the best instead of having a goal strategically or knowing when to take or seeing a tactic. Thanks for the input guys I know I have a lot of room for improvement playing about 100 games or so

There are several reasons to annotate our own games, particularly those we lost because we try to isolate the source of our mistakes to not to repeat them.
In your case, you're not paying attention to the threats. That's like playing baseball and not caring for where the ball is or where it's traveling to.
