A4 and H4 and bringing the rooks out immediately has got to be close to number one worst. The Kings gambit is a close second for me although many have good luck with it.

The worst opening is the one that you are least comfortable playing because you don't know the lines and have a tough time analyzing them.

Anything that aims to control the center is a good opening move. You want to avoid flank pawn moves because they essentially lose you a tempo. b3 can work though (the Nimzo-Larsen) because after Bb2 you have sustained pressure onto the center. The English and Reti opening work as well. For black I recommend the Sicilian as an answer to 1.e4 and the King's Indian/Grunfeld as a response to 1. d4. This is better in my experience than the Modern Defense.
