Actually, 3. ... Qxd5 leads to an inferior variation of the Scandinavian. White doesn't have to worry much about it because he can simply follow the usual Caro patterns to get a better position.
I'd say that you both made mistakes on move 19. Black's knight on d5 looks nice, but its usefullness is really questionable. Having exchanged it, you have given Black an opportunity to break free. But the recapture was made with the wrong pawn. After 19. ... cxd5! followed by b6, I cannot see any White's advantage. The rooks are placed oddly on e & f files, the bishop on c1 looks weird, while the beast on h7 suddenly becomes a beauty. Of course, the vices of Black's cramped position would not give him an edge, but the game would be much closer to equal.

And of course it's easy to find criticisms... I'm no GM even at 150 minutes for 40 moves - much less when playing 3 0 .
Already, if you take my invasion with Re7 - of course (I didn't see it during the game, only afterwards) he can go right away Qd8, and I have to remove the Rook... and then he can take on c5...
But sometimes you dare, and run away with it!!
In fast play, when you don't see the refutation (and I certainly didn't see ...Qd8 before playing Re7) - you just go for what you believe in, come what may :-)

More to the point - I think that your assessment is pretty correct. Much to the point, after 19...cxd5, his e-pawn would stay as a stabilizing factor on e6, which would mean no knight outpost on f5, no open e-file, no tricky knight forks on d6 and e7 so easily...
Maybe 19. Nce4 was a better move, continuing my no-exchange policy (of course he's invited to take with his Bh7, whereupon 20. fe would look pretty interesting.
My opponent played an unfamiliar capture on move 3 of the Caro Cann. We played three games with this variation.
In the first two, I had some horrible tactical oversights, but in this game I sacrificed my rook three times, taking into consideration three different circumstances - all involving an invading knight and a very checkable black king...
Twice he refused it, but on the third time he had to accept it, although it made him no happier.
