I don't think I played all that great especially in the beginning.  What could my opponent (white) have done to change the outcome of the game?

I should say that once he let my bishop take on move 24 he was down a rook at least unless he moved the king.  I think he risked mate though by doing that.

After move 11 was where I thought I would lose.  I was wide open.

Your King was really open after gxh6 and on move 15, instead of dxc5, things could have gone like this :

However ,after bxf3+ in the real game, the position is clearly winning for black.

In your scenario, I would have moved my rook to the F1 file instead of E1.

If you moved 2...Rf8 then Qxf8 is mate
