I think the crushing moment was when you allowed the e6-e5-e4 pawn push. You were doing well and had a clear plan, but then you allowed black to take space, make threats, and open up an excellent diagonal for his otherwise useless bishop. Instead of retreating your best minor piece on move 17, a better plan would have been to exchange central pawns and perhaps the f3/d7 knights and then work to plop a knight on d4.
