Not entirely but your opponent did helpfully stick his head in the noose with 20 Qxf3. After that I think you were OK apart from a couple of subpar moves, but even those didn't give white a lifeline. Before that it was a bit of a mess.
After 12. Qd1 black is so far ahead in this game it won't require best play to win. I probably would have played 12. ... g5, but that doesn't mean it's any better than your move, just a different idea. Instead of 13. ... Bxh2 and 14. ... Nf3, I probably would have chosen 13. ... Nxb3 14. Qxb3 Bxh3 (gxh3 is met by Qxh3 and mate on h2).
