What do you think about this Queen sacrifice?

Looking at your position, in the end, I think black will have a comfortable advantage heading into the endgame.  Your move Qd7! is a fine one that allows black to keep the piece, due to the fork you already pointed out, leaving a position which, if simplified, gives black has a queen and a pawn vs. a rook and a knight: I think it's safe to say here that black will have a significant edge heading forward.  All in all, I think white's sacrifice was a flawed one.

By the way; was this from an actual game and, if so, how did it play out?

No, I don't think so. It looks like the best White can do is to gain a rook and a knight in return for the queen, one pawn and otherwise inferior pawn structure. Also, no real opportunity for counterplay that I can see.
