Why did my attack die out?

I had a strong attack for nearly the first 35 moves (moves 27 -> 35 are very tactical), but then it died down. My opponent defended superbly. I still won becuase he blundered at the end.

 

What is your opinion on this game, where do you think I could have done better?

Why not 16. Nc7, winning the rook?

He actually played Rc8 on move 15. and Bg7 on move 16. This was a home game and I got the moves interchanged.  I corrected it now. Thanks PepeSilvia

Thanks for looking at it. Probably, the big mistake I made in the game was 21. Rd2, instead of h3.
