well i haven't really looked at the opening/mid game, but your endgame was extremely sloppy.

37. Kf2 seems to give up a pawn for no reason

at move 40 it seems moving the bishop and then attacking b4 pawn is a simple win

i think you flagged at the end here, 50. Nc5 seems drawn

7. d5! should be the right move

12. cxd5! followed by e4

in the end where is the loss?

As far as my limited understanding goes, losing the pawn on move 37 mortally weakened your king-side. This is why black's h-pawn seems unstoppable in the endgame. I suggest 37 should have been Kg2! This would have either assured an immediate material gain on move 39.

You certainly were winning, but you missed a very strong move on move 36. 36.d6+ Kg7 37.d7 Nfe6 38.Nf7! If Black takes the knight with 38...Kxf7 then you win with 39.Bxe6+

Doesn't the d4 knight also cover the e6 square? After 39.Bxe6 doesn't black have Nxe6?
