Chess Training From Titled Players

I am relatively new to chess and I do not possess any specific apptitude for chess. I am also not of the belief that titled players were born with an innate ability for chess that they alone possess. I think it helps, but I think discipline and hard work are what make them great. That being said can any titled players explain to me a step by step approach to chess training. There are many books on chess out there and I own quite a few. But very few, if any detail an incremental study plan. By incremental I mean when you were at 1200 what did you study versus what you studied at 1300 and so on. Also I know chess.com uses the glicko rating system which often rates us higher than ELO rating systems. Not saying which system is a better rating system. I'll leave that to the statisticians. I am just saying maybe an adjustment could figure into any training regimens. I also have noticed I have a hard time with chess visualization, ie calculating variations. I am just curious if there are in any commonalities in the way titled chess players have studied chess versus us class players. Whatever overlapping study regimens or habits I discover I will surely incorporate. Any help is greatly appreciated.

NM Dan Heisman also has some stuff related to that on his site I think.
