Converting Tactics to Gameplay

My tactics rating is currently 1450, and my training rating is 1500, yet I seem to be stuck below 1000 in blitz and below 1100 in rapid. Can anyone offer some advice on converting prowess in tactics to improvements in my game? I already watch professional chess frequently, read chess books, and solve puzzles daily, but I just can't seem to improve.

After looking at some of your games, i see the usual:

Hanging pieces.

Not following opening principles.

Not double checking your moves.

Missing simple tactics.

What i would suggest is this:

Play slower time controls, at lats G30 and longer.

Lay off the blitz/bullet, if youre serious about improving.

Keep on studying tactics.

@IMBacon Thanks for the advice? Is there anything more specific about my games that I could improve? I will start playing 30 minute games.

I know the feeling. What I'm trying to do is learn how to play positionally but look for tactics before each of my moves. Learning positional play is hard for me though. :(

Keep on studying tactics.  If you study tactics online.  Give yourself 2-3 minutes to solve the tactic.  If you cant solve it in that amount of time, setup the position with a real board with real pieces, and try and solve it.  Studying in 3D compared to 2D can and will make a difference.    

Review your games first, and then run them through an engine.  You will start to see a pattern of where youre going wrong.  Ideally, get someone stronger to go over your games with you.  

Double check your moves.  Take an extra few seconds to do a double check, and make sure your pieces are safe.  

After each of your opponents moves, ask yourself:  "What is my opponent trying to do?"

One of the most important things, that will sound like a "DUH" moment.  Take the time to look at all 64 squares. 

Look for Forcing Moves first:

Checks

Captures

Threats

Calculate out all lines as deep as yu can, even the bac lines.  This will force you to "see the entire board".

All of these things are difficlut to do in short time control games.  That is why you should play longer time controls.
