general chess survey

Hey chess.com users; I am conducting a general chess survey, and it is anonymous. I'm comparing perceived level of skill (in chess) vs. frequency of play, for my school. The survey's only 5 question's long, I'd really appreciate your time to fill these out for me....

2. Which phrase do you agree with most: In a regular week, I play chess for:

3. Please rank the following venues from 1 to 4, 1 being least frequent and 4 being most frequent, in which chess is played:
    _ a physical location       _ online   
    _ tournament           _ correspondence

4. 1 being the lowest and 5 being the highest, please rate the following ways of developing the level of skill in chess; the same value can be used more than once:
    _ reading chess books        _ playing chess against a human
    _ watching others play         _ playing chess against a computer

5. In a scale of 1 to 5, level one being lowest and level 5 being highest: I perceive myself to be a:
    _ level 1 (pre-junior)
    _ level 2 (junior)

I think it might help if u defined those categories.

Yeah, those seem more like age groups than skill levels.

The demon( i.e., my computer) wont allow me to input anything???

Also it seems like you left a space for each one.  e.g. a person really thinks they're a middle (5) but maybe a senior so I'll put a 4 there, and maybe a junior too, so maybe a 4 there also.

So instead, shouldn't the directions be to choose one of the following (then give the list)?

1). Make sure the monitor is within arm's reach.

2). Pencil is no good.  Pen is nearly as bad.  Try a sharpie.
