Read Silman's Reassess Your Chess.  It will definately get you over your hump.  Take your time reading it.  It gives you plans on what to do in the middle game.  It teaches you what to watch for, loose pieces, holes, solid squares to get you N and/or B to. . . much more.  I'll sell you my copy for $8 shipped.  PM me.

Here tips from Grandmaster Susan Polgar:
1.Control the Center!
2.Develop your pieces as soon as possible!
3.Castle as soon as possible!
4.Do not bring out your Queen early!
5.Do not move the same piece twice, before you have completed development! (unless it is being attacked or you can capture something for free).

You can never go wrong with the book DavyWilliams suggested. Also, it would help if you didn't waste time on chatrooms claiming "Sven" is an odd name. Hit the tactics trainer instead. ;)

Coming from a total noob, dont listen to me..nevertheless.. i have played chess for about one year and few months, With learning tactics i reached 1300-1400 quite quickly, played only blitz and bullet, then i started to go suddenly very much backwards, like i was all the sudden playing more worse than before, i lost actually interest in the game.. Then stopped playing blitz and bullet totally. I had a feeling it was harming my chess "thinking".  I felt there was no time for deeper toughts, originality and learning. I started playing only longer time controls and started to read more about strategies and endgames etc. and had like a reawakening to this incredibly deep and complex game...And, yes, what was my point.. Hmm.. nevermind..

Not nevermind. Everything you said was spot on! I went through that as well, although I can't help but play blitz from time to time. It's just too fun. ;)

Being happy and enjoying your chess helps tremendously, this concerns all aspects of life and its challenges, you don't have to ever be content with what you have or know, be grateful but hope and aim for more...

Seriously - take a longish break! I just did it having hit a plateau of 1500 standard, and it really helped. I wrote about it here; got some helpful comments on the subject too. And - stop playing blitz! ;)

You shouldn't try to quantify your progression, the focus should be on improvement once you begin to numerate these objectives you either limit your progress or hinder your progress due to an unlikely successful attempt.

To improve at standard (chess in general) is having the ability to identify a positive reinforced move which supports your previous move... In other words your choice of moves such be logical and should help build an attack or defence or trap etc. Once you identify moves which may improve your aim (attack/defence) you should analyse the strength of this move, briefly look over the board to see if your move weakens your pawn wall or disrupts your set up of pieced or opens up weak spots for your opponent, once you do this assess whether this move is worth the risk and you have good compensation, then decide whether to stick with this move or find another logical tactical move.

Either add attackers or defenders etc.

Depending on how many choices you have and how well you are able to identify threats or weaknesses will determine your strength at standard. Play for quality and not for time

It will take time to improve your "vision" I am always trying to do this in each game.

It would help if you looked up basic attack principles ie how to effectively use pawns, how to initiate a minor attack, using diagonals effectively, effective exchanges, sacrifices etc.

I'm very similar to where you are.  Stuck in the upper 1300's and low 1400's for months.  I recently saw some better results in my standard time games 15/10.  Rating moved up to 1550.  I've moved away from blitz games because I am trying to play using the ideas from the books I read this summer (two of Silman's books and one from Gambit on e4 openings).  Blitz does not give me time to process and apply the knowledge but 15/10 does.

Spend lots of time on the tactics is my suggestion.  Your vision will get better with hard work.  Then you will start to see how tactics hold your position so you can use the extra tempo (instead of using the move to defend) to build up. 

I just started annotating my own games and I think that it is a really beneficial exercise.  You get to play less, but I feel like it reminds me of what I studied and how I applied it to better my position or failed to apply it and how my position diminished.
