How do you really get better in tactics? Help needed

A 70% success rate is very impressive. I've never used Chesstempo; are those problems timed the way they are here on the Tactics Trainer? If not, then that might be part of the problem. Like almost everyone else who use the Tactics Trainer I have complained bitterly about the timer, but maybe the sense of urgency that it conveys is necessary to approximate game conditions. Just a thought; I could be wrong (and often am ).
I am completely disappointed in my tactical ability. I followed the advice which is told to every new players: learn tactics.
I have read books about it, solved problems in CT-ART but my main training tool was chesstempo. I have solved nearly 5000 puzzles with a 70% succes rate. However when I play live games I still miss attacking possibilities and lose games because I don't recognise the opponent's threats in time. I feel like I'm nearly at the same level even after a couple thousands puzzles.
Obviously I'm doing something wrong. What is the right approach of learning tactics? If you improved a lot tell me how did you train yourself please, share us your experience. Thanks
