How to understand time controls?

I have been playing chess for about a year and a half now and I want to play at a club, but for the life of me I can't figure out what 30/90, SD/60, d5 time control means. Could someone explain it to me?

30 moves in 90 minutes (1-1/2 hours for 30 moves) Sudden Death in 60 minutes, game ends in an hour after first time controll. The will be a 5 second delay per move. If you finish 30 moves in 90 minutes you will receive the remaining time plus 1 hour to finish the game. If fail to complete 30 moves in one and a half hours you will lose, if you fail to complete the game with remaining time and one hour after move 30 you will lose. If you move with in 5 seconds it will not cost you any time.

Note:  It's lose, not loose.  Loose is when something isn't tight.  "Her shoelaces were loose."
