I think Watson, Schandorff, Kornev, and Kaufman advocated 1 d4 d5 2 c4 e6 3 Nc3 Nf6 4 cxd5 exd5 5 Bg5, while Burgess advocated 1 d4 d5 2 Nf3 Nf6 3 c4 e6 4 Nc3 Be7 5 Bf4. I think some of these authors have written advice for White on dealing with the KID.

Correct...The subtitle of the book is "Winning With 1.e4"....
It is a popular openings repertoire book for the beginner-intermediate player...
Agree. Sadler's book is great.
Is there a good book for Black against 1. d4 d5 and 2. everything except c4 (for example Colle, London, Catalan etc ...)?

If you include DVDs and videos, then you might want to check out GM Simon Williams's site, gingergm.com. He has a complete 1.d4 repertoire for the attacking player, where he covers every major line from the white perspective. He has a good mix of memorized moves (play this or you'll lose, so memorize it) and also general commentary (such as, 'in these types of position, our goal is always the d5-break,' or 'do not be in a hurry to win back the pawn, because our compensation is long-lasting').
I'm an 1.e4 player with a preference for slower, positional games, but watching the first half of this series has seriously gotten me considering switching. If videos are your thing, it's worth a shot.
I am a d4 player and I am looking for suggestions on good d4 opening books.
Particularly I want a book on the queen's gambit, a book on the KID, and a book that covers 1.d4 more generally.
