Shorthand Chess Lingo?

I've noticed, whilst reading anything about chess, that people tend to write in a shorthand that I don't understand. For example, 1. e4 c5 2. Nf3 d6 3. d4 cxd4 4. Nxd4 (Nc6 5. Nc3)  

WHAT DOES THAT MEAN?

Can someone explain to me how all this works, not just definitions for the example above but all the rules for talking and reading this 'chess code'.
