They are two distinct tactical motifs. The difference is that, with a fork, one piece is responsible for both threats. With a double attack, two or more pieces are involved. So Loomis' example is a double attack, as both the Queen and the bishop are involved.

I think a fork is a direct threat on two or more pieces, whereas a double attack is an attack on two things (e.g. threatening backrank mate and a loose piece)
