He just surfaces 5 minutes later under a new account. So until chess.com complicates the account making process and severely limits new members' posting abilities -which is something they seem most unwilling to do- deleting said acounts is spitting into the wind.
I have explained before the security we used on a site where security was paramount.
We could ban by username, email address, IP address, and hostname. We also banned every problem proxy we encountered.
In time, the spammers, scammers, trolls, and troublemakers realized they were fighting a losing battle.
Just do deep packet inspection on the network level and drop all packets containing "baba ji", "kala jadu", "gold medalist magician" or other nonsense even before they hit the servers.

On another site with which I am associated, the button was located discretely in the bottom right hand corner of each page, and it was not activated by hovering your cursor over it.

You are probably correct. This is a profit-oriented site. What will ultimately motivate the administration of the site is the thought that trolling equates to lost revenue.

I don't want to believe that, but the complete lack of initiave in this area tells me something.... and it's most definitely NOT that nobody knows about it.

I usually see at most one GM in live chess at any time.
One GM out of the existing 2,000 or so? When they live in different time zones, some don't play internet chess, etc.?
It would be pretty lucky to have one GM in live chess at all times.

Soon all public librarys will eventually be IP banned, cafes, any public wifi. Not to mention ligetimate users who get assigned dynamic IP addresses will accidently be banned. Ulitimately banning a bunch of IP addresses will start killing business.

In the past we reported the exact ip address and time of the incident, and let them handle it in their end. A public library or university does not want their facilities used for criminal activity.
If there is the will to combat people who make threats on the site, there are ways to do it effectively.

I'm sure your site was smaller and probably not as international. The tactics you used just don't scale well to a site the size of chess.com. Yes, report death threats to the hosts repsonsible and let them deal with it on their end but I think some of these proxy services just won't do anything, other sorts of abuse like cheating or just being spammy will probably recieve no response at all. There are ways to reduce abuse but blocking a bunch of IPs will hurt chess.com's business model.

I agree in part. Playing host to criminals will ruin the business model too.
