Joking aside, I don't think I'm really "scared" of anything. I'm not just saying this. Of course I'd be concerned for safety or things in specific situations, but this is more of an awareness than an actual "fear" I feel like.
I'm not sure what my biggest "fear" would be if I thought about it. Hmm. This might sound philosophical, but I think my greatest fear would be being unable to help a loved one; this helplessness seems like something I might fear.

@KeSetoKaiba not at all. My wife has chronic migraines and there hasn’t been a cause of it diagnosed yet. She’s getting treated for them, but she still gets debilitating migraines every so often. The fact that there’s nothing I can do for her eats at me.
Woolly has a fear of being neutered , and losing his best friends ! 🙀

Likewise, I've had a few similar experiences too and it makes one feel so powerless because it feels like there is nothing we can do 
p.s. I also get migraines (although only occasionally and not chronic), so I can relate somewhat. If your wife's migraines are similar to mine (they might or might not be), then probably just your comfort/concern for them helps some - even if more just emotionally than anything else 
For what it is worth, sometimes fresh air or water help migraines (not always) and I've also found that sometimes a sip or two of a soft drink (coca-cola, pepsi etc.) can sometimes help as well. I'm not sure why that helps (carbonation doesn't sound like something which would help etc.), but sometimes it does. Those things won't work every time, but they work enough of the time that I typically try that when I have migraines. If you wife hasn't used one of those (fresh air, water, or coca-cola/pepsi), then it might be worth trying.

Im not really scared of anything in the world. I just fear the afterlife. So basically, I’m scared of dying.
