Google ads reviews ads before they are posted up. In the sense that they mindlessly accept everything unless their AI tells them otherwise. Even scams are let through sometimes. But hey, Google makes plenty of money! Yeah it's trash. I've seen M rated tv show ads on kids channels (youtube).

The site uses a third party ad provider and doesn't directly approve ads. They can define categories and types of ads but sometimes things slip through the provider that are not categorized correctly.
That ad does not look inappropriate. As to ages on the site, there are a lot of adult members on the site and by default, anyone under 13 should not be here without parental approval and monitoring.
For a really bad ad, you can always report.
