I don't know where I lost the game

This is game is the last round from WA open where I won the first three games and failed to gain even 0.5 points in the next three rounds :(

My opponent played this game pretty differently, I thought I was holding on the game and have a lot of fight but I lost the game pretty suddenly. My position became hopeless.

Update: I have created my analysis for this game at - http://blog.chess.com/smalugu/strategic-mishandling

    I think 20 ...a5 was not right.

    To open a line how about  20 ...a6 , followed by ...Nc8 and ...b5

@Xanatos thanks for the Rybka analysis, I now see some places where I could've improved upon my play. But what I am actually hazy about is to know the reason why black has been condemned to a life of passivity and defense through out the game. My judgement call to get a pawn phalanx on e5 and f5 and thinking that bishop on h3 is merely a spectator was dubious. Today I would add my strategic analysis to this game.

You were not aggressive enough.  On move 5 you moved to d7, you should have taken the pawn, then exchanged queens.  Instead you retreated until he forked you in the end.
