That annoying queen is in the center for one reason: You brought it there! Don't play 4...Nxd4. Play 4...Nf6 or 4...Bc5

I assume this is the position you mean, and I must ask how is this a non-exchange? White can still play Nxd4 after Bc5.

The point of the Malaniuk is to force c3 to displace the b1 knight. After Nxc6 Qf6 and the ensuing trades, White either has to play Nd2 instead of the much stronger Nc3 or play c4 which both gives back the tempo and plays c4, not usually a great idea in the Scotch. It’s the positional approach, as the “best” way to play against it is to play a normal Classical up a tempo, but Black is 100% okay with that.
