Italian game help?

I have a few questions on the Italian game. 1.e4 e5 2.nf3 nc6 3.bc4 bc5... Background information: I know a lot of the lines well, including lolli attack, fried liver, mainline guicci (4.c3) and the max Lange attack. Problem: After playing  1.e4 e5 2.nf3 nc6 3.bc4 bc5. I occasionally decide to attempt the max Lange attack.  Therefore 4.d4 exd4 5.0-0 - this is where I require nf6 to get into lines I know, however, everytime, they seem to play ...h6. I cannot find one game where any respectable oponent via FIDA ratings ever face h6. Is it an error? What move should I play? I seem to find myself doing 6.re1 hoping they would fall back into lines I know. Help please?

i would recomend c3, and if d4xc3, Bxf7+. if king takes:Qd5+, and taking his bishop on c5 next move, where if the queen is kicked it may take the pawn on c3, if not then Nxc3, if cxb2: Bxb2 with a comfortable position. back to the begining if black doesnt take on c3 then cxd4 with a strong center. or- if Qf6 (blocking the development of his knight to try to hold the pawn) e5: with threat-> Re1, to bear in mind.

Yeah, 6 c3 looks promising to me too.  But what's wrong with your 6 Re1?  Now Black will no longer have the 6... Nf6 7 e5 d5 idea available; and if 6... d6 7 e5 could be a bit of a nuisance.
