I use the London System against 1...d5, I hate playing against Queen's Gambit Accepted, I sometimes use it against 1...Nf6. There is a book called Ideas Behind the Modern Chess Openings by Lane that covers the London System including its use by Kasparov.

It depends. If your opponent is playing 2. Nf3, then 2...c5 is pretty darn effective because to get the best play he will have to switch out of the London.
This is actually the reason why "Winning with the London" builds its repertoire around 2...Bf4, which is more committal but allows London play against a vaster variety of setups.
