I answered yes to both... but this is a pretty blunt instrument... are you 12 years old?
I haven't cheated in a rated game... and the games I did use computer assistance in had a particular reason to do so... I get the feeling you'll draw some pretty ridiculous conclusions from a 2 question survey like this.
I am above 50 years old, and think that results from such a survey can be interesting for many people in the chess communities. So far several hundred people have answered in one day. The link to the survey is also posted in lichess.org (forum there). And I will post it in reddit, in the biggest chess forum there. The goal is that at least ten thousand players will answer.
