12. ... Qd7 was not great, but black's position is already cramped and inflexible. Surely his downfall came before moved 12.

 

21. b5 was not the fastest way to win. Better is 21. c5. This will force open lines because black is forced to take the pawn, he can't allow you to play c6. Then your rooks will crush him on the open b-file.

 Yes, it is, but IMHO after 11... O-O black's position posible to resist.

Totally brutal.  I have played the black side of that game many times.

White created a nice central crush. Black did not develop. The queenside bishop and knight moved early, while the kingside was totally neglected. Maybe Black had to try g5 on move 14 or 15.
