1. d4 d5 2. Nf3 Nf6 3. Bf4 e6 4. Nc3

In the Double Queen's Pawn openings, it is generally considered best to not play Nc3 until the c-pawn has advanced so as not to block it.

4. ... Bb4 5. Qd2 (?!)

I don't like this move - see my comment for the following move - I'd probably have played 5. a3 instead.

5. ... O-O

Here I think black should have played 5. ... Ne4! which would gain a tempo on your queen and allow 6. ... Nxc3 7. bxc3 Ba4 doubling your pawns with no compensation (that I can see).

6. O-O-O Nc6
(?!)

Here 6. ... Ne4! looks even better for black here than on the previous move: 6. ... Ne4 7. Qe1 Nxc3 8. bxc3 Ba3+ with c6 and Qb6 to follow looks like big trouble for you.

7. Bg5 (!?)

This move looks pretty good to me. Even though it goes against the opening principle not to move any piece twice until all your pieces are developed, you've, at least, made Ne4 less attractive for black since 7. ... Ne4 8. Bxd8 Nxd2 9. Rxd2 and black can now only double your pawns by giving up the bishop pair.

7. ... Bd7 8. Qf4 Bxc3 9. bxc3 b5 10. e3 Rb8 11. Ne5 (!)

A nice agressive move treatening to win a pawn by diverting the black queen after Nxd7

11. ... Nxe5 (?)

looks to me like 11. ... Be8 instead holds the pawn.

12. dxe5 h6 13. Bxf6 gxf6 14. Qxf6 Qxf6 (?)

Here 14. exf6 followed up with Qxh6 and Qg7++ looks crushing.

15. exf6 c5 16. e4 (!?)

Laudable that you saw the pin and this idea. This move gives your rooks more scope by threatening to open the d or e files. It also makes possible e5 supporting the weak f6 pawn.

16. ...Bc6 17. exd5 Bxd5 18. Bxb5 (?)

This just loses a piece. 18. Kb2 instead.

18. ... Rxb5 19. f3 Bxa2 20. Rd7 (?)

Overlooking a simple skewer. You might want to concentrate studying on tactics. Tough to find a good move for white, but 20. Kd2 looks reasonable.

20. ... Rb1+ 21. Kd2 Rxh1 22. Rxa7 Bc4 23. Rc7 Rxh2 24. Rxc5 Rxg2+ 25. Ke3 Bd5 26. c4 Rxc2 27. cxd5 (?)

Really have to watch those tactics. This time, it's a pin - 27. Kd3 instead.

27. ... Rxc5 28. resigns

Hope this analysis helps.

You might try the website www.chesstactics.org

I ran my analysis through the computer, and I did make a couple errors - not too bad though, I was mostly correct. I'll get back later with an update.

SisyphusOfChess

Here, as promised, are my analytical errors.

1) On move 11 for black, I recommended 11. ... Be8?. But then white can play 12. Ng4! attacking the f6 knight again and threatening to open up black's king position. GNU-chess' 11. ... Nxe5 was actually better than my recommendation, but my computer thinks 11. ... Qe7 black's best.

2) On move 6 for black, I said 6. ... Ne4! was the best move. I still think this is true, but the line I gave "6. ... Ne4 7. Qe1 Nxc3 8. bxc3 Ba3+ with c6 and Qb6" isn't quite so bad for white as I had thought. He can just play Ka1 and, though worse, there doesn't look to be any immediate win for black.

SisyphusOfChess

You don't play too badly. You actually come up with some really good moves. Just remember not to take anything unless you get something of equal value or more in return, and watch out for those tactics.
